Overview: We are seeking a highly organized and compassionate Support Coordinator to join our team. The Support Coordinator will be responsible for providing assistance and guidance to individuals in need of support services, coordinating resources, and ensuring that clients receive the appropriate care and assistance tailored to their specific needs. The ideal candidate will have excellent communication skills, a strong ability to multitask, and a passion for helping others.

Key Responsibilities:

  1. Client Support: Provide direct support to clients by assessing their needs, developing individualized support plans, and connecting them with appropriate services and resources.
  2. Coordination: Coordinate with internal teams, external agencies, and community resources to ensure seamless delivery of services and support to clients.
  3. Advocacy: Advocate for clients’ rights and needs within the organization and with external stakeholders, ensuring that they receive the highest quality of care and support.
  4. Documentation: Maintain accurate and up-to-date client records, including progress notes, service plans, and relevant documentation in accordance with organizational policies and procedures.
  5. Crisis Intervention: Provide crisis intervention and support to clients experiencing emergencies or challenging situations, ensuring their safety and well-being.
  6. Education and Training: Provide education and training to clients and their families on available support services, resources, and self-advocacy techniques.
  7. Monitoring and Evaluation: Monitor client progress and outcomes, evaluate the effectiveness of support services, and make recommendations for improvements as needed.
  8. Team Collaboration: Collaborate with colleagues across departments to ensure a holistic approach to client care and support, sharing information and coordinating efforts to meet client needs.
  9. Compliance: Ensure compliance with all relevant regulations, policies, and procedures governing the provision of support services, maintaining confidentiality and adhering to ethical standards at all times.
  10. Professional Development: Stay informed about best practices, emerging trends, and developments in the field of support services, participating in professional development opportunities to enhance knowledge and skills.
