Full job description

Australian Disability Care Services (ADCS) is currently seeking a compassionate and dynamic Team Leader to oversee the provision of disability support services. The Team Leader will play a pivotal role in leading and supervising a team of support workers, ensuring the delivery of exceptional care and support to individuals with disabilities.

Responsibilities:

  • Leadership and Supervision:
    • Provide strong leadership, guidance, and direction to a team of support workers, ensuring that they are equipped with the necessary skills, resources, and support to deliver high-quality care and support services.
    • Supervise, mentor, and motivate team members, fostering a positive and collaborative work environment that promotes teamwork, communication, and professional growth.
    • Conduct regular team meetings, performance reviews, and one-on-one sessions to provide feedback, address concerns, and identify opportunities for skill development and improvement.
  • Service Delivery Oversight:
    • Oversee the day-to-day delivery of disability support services, ensuring that services are provided in accordance with individualized care plans, organizational policies, and relevant regulations and standards.
    • Coordinate and allocate Residents/NDIS participants support schedules, ensuring adequate coverage and continuity of care for all NDIS participants.
    • Monitor service delivery outcomes, NDIS participants satisfaction, and adherence to service agreements, and implement corrective actions and improvements as needed.
  • NDIS participants Care and Support:
    • Collaborate with NDIS participants, their families, and other stakeholders in the development and review of individualized care plans, ensuring that services are tailored to meet the unique needs, goals, and preferences of each participant.
    • Provide direct care and support to NDIS participants as needed, including personal care, meal preparation, medication assistance, and other activities of daily living, while promoting independence and dignity.
  • Documentation and Reporting:
    • Ensure accurate and timely documentation of Participants records, service plans, incident reports, and other relevant information, in accordance with organizational policies and regulatory requirements.
    • Generate and maintain clear and concise records of participants progress, interventions, and outcomes, and communicate relevant information to appropriate stakeholders as needed.
  • Team Collaboration and Communication:
    • Foster positive and effective communication and collaboration within the team, as well as with NDIS participants, families, allied health professionals, and other stakeholders.
    • Act as a liaison between the team and management, relaying feedback, concerns, and suggestions for improvement to ensure the continuous enhancement of service delivery and team performance.
