Home Transition Support
What it is and who it's for
Leaving hospital is a big step, and getting home safely often takes more coordination than people expect. ADCS provides Hospital to Home Transition Support for NDIS participants preparing to be discharged and return home, or move into another living arrangement, across Western and South-Western Sydney.
This support is for anyone whose NDIS plan includes funding for home and living, daily personal activities, or life stage transition supports, and who wants help making discharge day — and the weeks that follow — go smoothly. You stay in control of every decision; we work at your pace and to your preferences.
How it works
- We liaise with you, your family or carers, hospital discharge planners, social worker, and your Support Coordinator (if you have one) to build a transition plan before you leave hospital.
- We help organise the practical side — personal care rostering, equipment, transport home, and any home set-up needed for a safe return.
- Once you're home, our support workers help you settle into a routine, and we adjust the plan as your needs change.
What you can expect
- Coordination between hospital staff, your Support Coordinator or LAC, and your ADCS team
- Help arranging equipment, home set-up and daily routines ahead of discharge
- Qualified, screened support workers for personal care and daily tasks during your recovery
- Regular check-ins to make sure the plan is still working for you
